| Brief Detail: | Seeking a Nurse Practitioner/Transplant Coordinator to fill this role for post kidney. The NP will demonstrate a high level of expertise in the assessment, diagnosis, and treatment of the complex patients and their responses to actual or potential health problems. Assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. Provides direction, education, and leadership. Delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. Safely administers prescribed medications, identifies incompatibilities and/or interactions. Registered Nurse Transplant Coordinator manifests a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems, prevention of illness and injury, maintenance of wellness, and provision of comfort for the pre and or post transplant patient. |
